Dear Sir or Madam:

With the enclosed resume, I would like to make you aware of my background as a Personal Support Worker. I have applied excellent patient skills, attention to detail and organizational skills in various developmentally disabled, home health and long-term care environments.

As you will see from my resume, I completed the Personal Support Worker Certificate program through NAME OF SCHOOL HERE. I have also successfully obtained certificates in First-Aid and CPR. I have also obtained (LIST ANY OTHER TRAINING CERTIFICATES, ACCREDITATIONS, ETC). While working as a personal support worker in home health and long-term care (AND OR ANY OTHER SETTINGS) environments, I have consistently demonstrated my ability to work independently and also as a contributing member of a health care team, including but not exclusive to working with fellow personal support worker’s, nurses and physicians, all while providing the highest possible level of patient care.

In previous experience as a personal support worker I have monitored and recorded behavioural changes to assist the health care team in determining appropriate treatment and medication. I have created programs for individual and group activities. I have worked within a reporting environment and am familiar with paper and computer record keeping.
